waiver anD RELEASE

There are certain inherent risks associated with participating in congregate activities which cannot be completely eliminated. COVID-19 is an extremely contagious virus and is reported to be spread through the air from touching contaminated surfaces and from person-to-person contact through droplets of saliva from the mouth. There is no known treatment, cure, or vaccine for COVID-19 at this time, which can cause severe illness and death. Federal, state and local governments have recommended and/or require precautions, including, but not limited to, social distancing and wearing face coverings to lessen the spread of this virus. Please contact the Centers for Disease Control (the CDC), your state or local governments for the most up-to-date information and guidance.

ASSUMPTION OF RISK / WAIVER OF LIABILITY / RELEASE / INDEMNIFICATION by couples and their guests, I KNOWINGLY AND FREELY ASSUME ALL RISKS related to our participation in a congregate activity (wedding) and understand that our attendance at such activity increases the risks of my being exposed to and/or contracting COVID-19. We have informed all of our guests of the inherent dangers and the inability of the Venue to maintain proper social distancing if there are greater than 10 guests in the Venue at any one time.

I also AGREE to forever RELEASE and INDEMNIFY THE PHILADELPHIA WEDDING CHAPEL AND ANY OF THEIR AFFILIATES, DIRECTORS, OFFICERS, OFFICIALS, EMPLOYEES, AGENTS OR VOLUNTEERS (the “Venue”) if we or any of our guests contract any illness (including, but not limited to, Covid-19), succumb to hospitalization and or death, or any experience any other loss or damage to person by participating in this congregate activity.

I understand that we are giving up our rights to bring any claims or actions for personal injuries, death, disease, damages, or any other loss, including but not limited to claims of negligence, whether known or unknown, foreseen or unforeseen against the Venue. I further AGREE to indemnify and hold harmless the Venue from any claims arising from me, us or any of our guests, individually or collectively that were infected or contracted any illness (including, but not limited to, Covid-19), death or any other loss or damage to person or property and shall be liable to pay attorneys' fees and costs incurred by the Venue to defend such claims.

We have informed all of our guests of the above mentioned inherent dangers in attending our wedding.

WE HAVE READ THIS WAIVER AND RELEASE. WE UNDERSTAND THAT WE HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT. WE ARE SIGNING THIS WAIVER AND RELEASE ONLY AFTER A CAREFUL REVIEW OF ITS CONTENTS.
